To make a referral to the Witness Service, for yourself or on behalf of someone else, please fill in our referral form.

You can use the form if you are a:

• a witness

• a family member or friend of a witness

• someone who needs support in court because a person you know has died

• or a professional from an organisation referring someone for help from the Witness Service.

The form allows you to tell us how best to support your needs, or the needs of the person you’re referring.
